Requirements

RECORDS REQUEST

EMAIL

  1. Fill out the application form { http://bit.ly/JIISrecordsRequest}
  2. Send the scanned copy of payment to [email protected] if not uploaded in google form
  3. Pick up your request as indicated in the tracking request form

MAIL

  1. Send a written request with the following information: Complete Name used when last enrolled in JIB, Year of last enrollment, Degree, Number of Requested Copies, Mailing  address and contact number and a copy of valid ID
  2. Staff will contact the requestor upon receipt of documents
  3. Deposit payment online
  4. Pick up your request as indicated in the tracking request form

OVER THE COUNTER

  1. Secure the application form at the Registrar window.
  2. Fill out the application form completely
  3. Submit your accomplished for at the Registrar Staff for validation 
  4. Pay fee at the cashier, Ground Floor, Accounting building. Open from 8am to 5PM. Lunch Break 12am to 12PM
  5. Return back the accomplished form and official receipt at the Registrar Office
  6. Get your claim stub
